      Hi Michael,

To request an application form by mail, send a self addressed stamped envelope to River Crest Apartments c/o Wavecrest Consulting LLC, 87-14 116th Street, Richmond Hill, NY11418.

Norwood News does not have any application forms. We are just the messenger. Alternatively, you could also call New York City Department of Housing, Preservation & Development to ask them to walk you through the online process. Basically, you just go to where it says “How to Apply” and then under where it says “Apply Online,” click on “Apply Now.”

      Good Luck!
